About Us

Community Health Action Network (CHAN) is a nonprofit organization founded in 2007. We provide a variety of programs related to health, education, and finance, to residents of the Victor Valley region of California. With a model based on client participation, CHAN is focused on assisting clients to invest in themselves and positively change their own circumstances.

Rather than establishing another “giveaway program,” the CHAN model is based on rewarding clients for becoming actively involved in our programs and events. Community members who regularly volunteer and attend sponsored activities may receive more generous assistance or employment opportunities as they become available. 

This more dynamic process of community assistance benefits our clients in three ways:

Volunteering and community involvement activities can be used to enhance the client's resume, moving them closer to stable employment.

Rather than a single assistance award, clients receive education in fundamentals designed to help them make informed health, career, and financial choices.

Disenfranchised members of the community can benefit from becoming advocates and building civic relationships creating greater awareness of how to facilitate change in their lives and neighborhoods.

Components of the

CHAN Service Platform

CHAN operates through a network of community partnerships with banks, schools, health institutions, nonprofits, and government agencies. Through these relationships, we are able to advocate for the vital services that our clients need for self-improvement.

Because our client services model is based on participation, we are able to encourage disenfranchised populations get involved in their communities. This form of dynamic engagement is vital to improving our community.

Information is often power. That is why we use a variety of training programs and classes to help our clients to empower themselves. Frequent topics include health and wellness, financial literacy, and GED preparation.

Democracy depends on the ability of people to advocate for their own interests. That is why CHAN is heavily involved in providing residents with opportunities to speak directly with civic leaders and businesses about issues of concern.

Whether the goal is gainful employment or entrepreneurial activities, CHAN's workforce development programs can get people on track to financial stability.

In addition to efforts to assist the entire High Desert region, CHAN is also committed to addressing the specific needs of Black families. This includes programs, events, and training opportunities designed to counteract the centuries-long impact of racism on the African American population.

Participation Model

Our service model is designed to reward our clients for remaining involved with the organization.
CHAN is a non-profit supported by grant funding and donations. All applicants must complete an intake form at the time of request for services. Initial requests for emergency assistance will be accommodated based on resource availability.
Upon subsequent requests for assistance, clients will be provided with opportunities for volunteering or attending CHAN-sponsored events. Participants who volunteer or attend such activities will be given preference when requesting emergency items like food assistance, baby supplies, or gift cards.
The more clients invest in the program, the more they will be rewarded. Community members who demonstrate dedication to CHAN programs may also be considered for employment opportunities with our organization or our partners.

Participation Model

Our service model is designed to reward our clients for remaining involved in the programs. Although anyone can apply for our services, we grant priority to families with children under the age of 5.
All applicants must complete an intake form that must be kept up to date. Initial requests for emergency assistance will be accommodated to the extent that we have the supplies on hand.
Upon subsequent requests for assistance, clients will be given information for volunteering or attending CHAN-sponsored events. Residents who volunteer or attend such activities will be given preference when requesting emergency items like vouchers, food, baby supplies, or gift cards.
The more clients put into the program, the more they can receive. Community members who demonstrate dedication to CHAN programs and services may even be considered for paid positions with our organization or our partners.
